Family Health Teams (FHTs) use a patient-centred approach to primary health care that brings together doctors, nurses, nurse practitioners, registered dietitians, pharmacists, mental health clinicians and other health-care professionals and support staff to co-ordinate the highest possible quality of care for patients – when they need it, as close to home as possible. FHTs are a key component of Ontario’s health-care plan to reduce wait times, improve access to primary care, and keep Ontarians healthy.
Mental Health Clinician
Bring your advocacy, risk assessment and management skills and knowledge of community resources, psychiatric disorders and human development, family life cycle and systems theory to this 0.9 FTE role with the Peterborough Family Health Team. Reporting to the Collaborating Physician(s), you will provide psychosocial assessment, short term psychosocial/psychiatric treatment and interventions, psychosocial health education, crisis intervention, and advocacy on behalf of rostered and referred patients as an integral part of a multidisciplinary team. You have a degree in a Social Work preferably at the master's level, you are registered with the appropriate regulatory body and possess at least 4 years of supervised experience in a counselling environment. Additional training and/or experience in Cognitive Behaviour Therapy, Mindfulness Based Stress Reduction, would be an asset. We are looking for someone with a comprehensive bio-psychosocial approach to mental health assessment, clinical formulation and treatment planning. You must have the flexibility to function within the unique nature of a primary medical care office setting, and be able to participate in, or implement, health promotion, education and self-management strategies.
